Thank you for reaching out to McFadden Bushnell to conduct your estate, elder law or special needs planning. For some of you this may be the first time you have put together your planning documents. For others, you may be updating documents you put in place some time ago. No matter which, we look forward to speaking with you about your estate, elder law, or special needs planning needs and goals, and helping to put the documents in place to accomplish your goals.

Preliminary Family Profile through DecisionVault :

Prior to our initial meeting we ask that you complete our Preliminary Family Profile through DecisionVault at the link below. We would ask you to please complete this at least 2 business days before the appointment in order to give us a chance to review it before our meeting.

Because we would like to make our meeting as productive as possible, if the Preliminary Family Profile is not completed at least 2 business days before your scheduled appointment we may need to reschedule our meeting. Please complete as much of the form as possible as the more information you are able to provide, the more efficient and thorough our initial meeting will be. If you have any questions about the form, please do not hesitate to call our office for assistance. 

 It will be helpful if you also upload or forward copies of any currently existing planning documents (i.e. any power of attorney, will), as well as any other documents that you feel would be helpful. Those documents may be uploaded through DecisionVault, or, may be dropped of at our offices.

For our Young Adult Planning (age 25 and under), please complete our Young Adult Profile through DecisionVault at the link below. We would ask you to please complete this at least 2 business days before the appointment in order to give us a chance to review it before our meeting.

McFadden Bushnell’s New Office Address

McFadden Bushnell has a new office address! As of January 1, 2024, our offices are in the Tower East Building at 20600 Chagrin Boulevard in Suite 805, Shaker Heights, Ohio 44122. 

PARKING NOTE: There is an onsite garage with visitor parking. If you have a taller truck, you may want to park along the perimeter of the building, or in the 2-hour street parking available behind the building on Northfield Road. If you park in the back of the building you can enter the building from the lower concourse level.
Many of our appointments will continue to be conducted virtually as has worked well for so many of our clients. Our in person meetings and documents signings will now be at our new office.
