Thank you for reaching out to McFadden Bushnell to provide assistance for Estate Administration or Trust Administration for your loved one. We are sorry for your loss.

Initial Estate Administration (Probate) and/or Trust Administration Form through DecisionVault :

Prior to our initial meeting we ask that you complete our Initial Probate Form through DecisionVault at the link below. We would ask you to please complete this at least 2 business days before the appointment in order to give us a chance to review it before our meeting.

Because we would like to make our meeting as productive as possible, if the Initial Probate Form is not completed at least 2 business days before your scheduled appointment we may need to reschedule our meeting. Please complete as much of the form as possible as the more information you are able to provide, the more efficient and thorough our initial meeting will be. If you have any questions about the form, please do not hesitate to call our office for assistance. 

 It will be helpful if you also upload or forward the following documents prior to our meeting. These documents may be uploaded through DecisionVault, or, may be dropped of at our offices.

  • Death Certificate

  • Paid funeral bill

  • Last Will

  • Current Trust Documents (original trust or most recent restatement)

McFadden Bushnell’s New Office Address


McFadden Bushnell has a new office address! As of January 1, 2024, our offices are in the Tower East Building at 20600 Chagrin Boulevard in Suite 805, Shaker Heights, Ohio 44122. 

PARKING NOTE: There is an onsite garage with visitor parking. If you have a taller truck, you may want to park along the perimeter of the building, or in the 2-hour street parking available behind the building on Northfield Road. If you park in the back of the building you can enter the building from the lower concourse level.
Many of our appointments will continue to be conducted virtually as has worked well for so many of our clients. Our in person meetings and documents signings will now be at our new office.
